The general contact form requests your name, email address, subject, and message. We receive the information you choose to send so our team can respond and direct your inquiry. Calling or emailing us also provides the contact details and information you share.

Please do not send medical records, diagnoses, identification numbers, insurance details, or other sensitive information through the general contact form or ordinary email. Ask a coordinator how to provide information through an appropriate channel.

How inquiries reach our team

Website inquiries are delivered by email. The institute's caresinst.org mailboxes forward messages to the principal investigator's pi@caresinst.com mailbox. Hosting and email service providers process information needed to operate these services. A research inquiry does not itself authorize release of your medical records or enroll you in a study.

Technical information and other services

Website hosting and security services can process technical details such as an IP address, requested pages, browser information, and request times to deliver and protect the website. Images or links supplied by another website may involve a connection to that provider. An external site's own privacy notice governs information you submit there.

Your questions and requests

Contact privacy@caresinst.org to ask about information you sent, request a correction or deletion, or raise a privacy concern. Please identify the inquiry using minimal information; do not email identification documents or medical records. Our team may need to confirm your identity and determine whether applicable recordkeeping requirements affect a request.

Children and research inquiries
